OHSC Membership

There are 2 categories of membership in the OHSC:

1. Organizational ($100 per organization, unlimited number of participants per organization)
2. Individual ($30 per person) - this is for those individuals not attached to a member organization

The above membership fees will enable the Coalition to cover the costs of our province-wide teleconferences and meetings. Becoming a member will enable you to benefit from the information-sharing that occurs and to participate in planning the future work of the Coalition.

To become a member, simply complete the registration form below, and mail your cheque identifying that it is for "OHSC Membership Fee", and payable to the Ontario Public Health Association to:

Registering for Membership

Note: by completing this form, you will automatically be added to the OHSC listserv as it is the primary method of issuing communications to Coalition Members.

Membership lists will not be posted on our website. Since the purpose of the Coalition involves networking and sharing of information among members, it is understood that names and contact information will be shared among members and provided to others outside the Coalition for appropriate referrals.
